Countess of Altamira and Her Son Vicente Isabel by Agustín Esteve, ca. 1778

She is María Ignacia Álvarez de Toledo, marquesa de Astorga (1757-1795). Her son is Vicente Isabel Osorio de Moscoso, Count de Trastamara  (1777-1837). 

She married 11 April 1774, in Madrid, to Vicente-Joaquín de Osorio-de-Moscoso y Guzmán, with whom she had five children:

The lady on this painting is the same as the one on this painting here, holding Vicente’s sister María Agustina. There is a bust of her in the Museo del Prado, see this link here.

“María Ignacia wears a unique dress with a basque waistline set off with a frilly peplum and a (not off shoulder) scoop neckline framed by a double bertha while her skirt and over-skirt are not supported.”
